By checking the Assumption of the Risk and Waiver of Liability check box in the form, the undersigned User (whether an individual personally or an individual acting as the agent or representative of an organization or other entity and binding that organization or other entity) acknowledges receipt of and agrees to the attached Facility Use Guidelines. User agrees to be responsible for the proper care of The Bradbury Church of Christ facilities, and agrees to pay any damages caused by, or as a result of its use of the facilities. User further agrees to indemnify and exonerate and hold harmless the Bradbury Church of Christ from any and all liability, and from all claims for damage, loss, or injury, to members of the public, guests, invitees, officers, or employees, or the Facility of the user, or any other person, occasioned by, or resulting from the use of the Facility by the user.
User hereby releases, covenants not to sue, discharges, and holds harmless the Bradbury Church of Christ, the Church Minister, and the Church Trustees, as well as the Church’s employees, agents, and representatives, in their personal, professional, or company capacities, of and from all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating to User’s use of the Facility.
User hereby agrees to report any and all damage to the Church Minister or Trustees promptly.
